As a Nurse Navigator, you will:

• Build Relationships: Develop strong connections with physicians, healthcare providers, and community organizations to ensure seamless communication and patient care.
• Coordinate Care: Oversee all aspects of patient care, guiding them through their journey from diagnosis to survivorship.
• Educate Patients: Provide vital information on disease-specific guidelines and treatment options, empowering patients to make informed decisions.
• Enhance Quality: Foster successful outcomes by developing treatment summaries and monitoring follow-up schedules.
• Drive Innovation: Participate in educational programs and contribute to performance improvement initiatives within the service line.

What We’re Looking For

To thrive in this role, you should have:

• Education: A Bachelor’s degree in Nursing; a Master’s degree is preferred.
• Experience: At least three years of clinical nursing experience; leadership or coordinator experience is a plus.
• Licenses: Current Texas RN license or temporary license, BLS certification, and relevant specialty certification.
