Your Role and Responsibilities

As a Nurse Navigator, you will:

• Build Relationships: Develop strong connections with physicians, office staff, and healthcare providers to ensure seamless communication and patient care.
• Coordinate Care: Oversee all aspects of patient care within your service line, advocating for patients and their caregivers.
• Educate and Empower: Provide disease-specific education to patients and caregivers, enabling informed treatment decisions.
• Drive Quality and Safety: Participate in performance improvement initiatives to enhance patient safety and care quality.
• Foster Growth: Identify educational needs and implement programs for healthcare providers and community groups.

What We’re Looking For

To thrive in this role, you should have:

• Education: A Bachelor’s degree in Nursing; a Master’s degree is preferred.
• Experience: At least three years of clinical nursing experience; leadership or coordinator experience is a plus.
• Licenses and Certifications:
  • Registered Nurse - Texas State Licensure (or Compact State Licensure within 60 days)
  • BLS - Basic Life Support (AHA)
  • Specialty certification (e.g., ANCC or Oncology Nursing Society) is preferred.

Working Conditions

In this role, you may be required to:

• Wear scrubs and business professional attire.
• Be on-call during emergencies, such as disasters or severe weather events.
• Travel within the Houston Metropolitan area as needed.
