Description of Position :

The Housing Navigator assists participants with serious mental illness in obtaining and maintaining subsidized or affordable housing. The Housing Navigator provides case management, individual support, skills training, and outreach in the office, home, and community to participants seeking housing, and works closely with partner agencies including the local Public Housing Authority and community landlords.

Essential Functions :

• Effective communication skills, verbally and in writing.
• Conduct intake and orientation with participants new to affordable and subsidized housing programs, and clearly explain program requirements and eligibility criteria.
• Follow established rules and policies for applicable programs and grants.
• Assist participants in clarifying housing goals, completing rental applications, and establishing and maintaining residence in the community.
• Assist participants in applying for long term/permanent housing subsidy programs.
• Assist participants with locating appropriate housing.
• Complete necessary documentation using a computer in a timely and accurate manner, and file required documents in participant files. Includes keeping up to date with tenant leases, rent changes, available funds, request for funds, and tracking requests for documents and signatures.
• Enter encounters in electronic health record (EHR) including service plan updates and progress notes for applicable cases.
• Ability to establish and maintain effective relationships with participants, community, partners, and staff.
