Social Worker - Outpatient Cancer Center at Intermountain Health summary:

As a Social Worker at the Outpatient Cancer Center, I provide essential social services to cancer patients and their families, focusing on psychosocial needs, care assessments, and resources. I collaborate with a multidisciplinary team, engage in community outreach, and offer support related to financial, emotional, and educational challenges unique to cancer care. My role includes designing support groups, implementing policies, and ensuring a comprehensive approach to patient survivorship needs and care continuity.

Job Description:

Provides social services to patients in the care site and/or assigned unit.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Provides continuing care assessments and reassessments of cancer patients and their families to assist in development of their treatment and survivorship needs. Identifies psychosocial needs and access to appropriate services and resources.
• Participates in oncology interdisciplinary rounds and multidisciplinary care planning. Communicates and collaborates with medical, interdisciplinary team staff, patient, family, and community agencies to provide continuity of care.
• Provides community resources for psychosocial, financial, and educational needs of patient and family. Collaborates with outside agencies. Participates in community activities, community-wide in-services and training to promote psycho-social health.
• Provides education and information on advance care planning and advanced directives. Serves as a resource in the areas of financial support, disability eligibility, workplace issues, transportation barriers, mental health, substance abuse, domestic violence, child and elder abuse, and housing resources.
• Provides education on palliative and hospice care, and coordinates referrals when indicated.
• Assists in the development, implementation and annual review of policies, procedures and protocols related to Oncology Social Work.
• Promotes mission, vision, and values of Intermountain Health, and abides by service behavior standards.
• Performs other duties as assigned.

As the Social Worker for the Good Samaritan Hospital Cancer Center you will work with our patients in Medical Oncology, Surgical Oncology, Infusion, and Radiation Oncology to:

• Provide specialized oncology counseling in a setting that allows for meaningful therapeutic relationships and continuity of care throughout the cancer journey

• Develop expertise in outpatient oncology support (emotional, transportation, financial, etc) while collaborating with a multidisciplinary team. 

• Design and facilitate specialized cancer support groups, with autonomy to develop curriculum and approaches tailored to diverse patient needs

• Participate, track measures/outcomes, and report out on as a designated member of our Commission on Cancer (COC) and National Accreditation Program for Breast Centers (NAPBC).

Minimum Requirements:

• Master’s Degree in Social Work is required.
• Valid LCSW licensed in the State you are practicing in.
• If LCSW is not completed at time of hire, LCSW is required within 3 years and clinical supervision is needed during the completion process.

Preferred:

• Oncology Certification is preferred.
• Current LCSW license.
• One (1) year of supervised social work experience in a health care setting working directly with individuals is preferred.

Physical Requirements:

Interact with others by effectively communicating, both orally and in writing.- and -Operate computers and other office equipment requiring the ability to move fingers and hands.- and -See and read computer monitors and documents.- and -Remain sitting or standing for long periods of time to perform work on a computer, telephone, or other equipment.- and -May require lifting and transporting objects and office supplies, bending, kneeling and reaching.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience. 

$37.46 - $57.80
